癌症病人的 Vancomycin 藥物動力學

摘要

本文以 vancomycin 為例,目前文獻顯示癌症病人使用 vancomycin 因分佈體積和清除率的增加,因此應使用較高劑量以達臨床上治療效果,而我國癌症病人是否也有相同結果很值得探討,因此本文藉由某醫學中心回溯性蒐集95年1月至99年3月使用 vancomycin 之癌症和非癌症的成人病人,在第一次達 steady-state 監測其藥物血中濃度,並依據監測結果使用一室分佈模式分別算出分佈體積、清除率和半衰期等藥物動力學參數,比較兩者之間的差異性。

初步結果癌症病人共蒐集50位,非癌症病人64位,兩者在平均每日 vancomycin 劑量和給藥間隔並無統計上差異,然而癌症病人平均 vancomycin 波峰濃度和波谷濃度均較非癌症病人為低,並具有統計上意義 (p<0.05),進一步分析藥物動力學參數,發現癌症病人平均分佈體積為51.22±13.11 L,高於非癌症病人42.66±11.32 L (P<0.05);清除率方面同樣癌症病人平均清除率為3.76±1.41 L/h,高於非癌症病人3.08±0.89 L/h (P<0.05),然而平均半衰期兩者並無統計上差異,此初步結果顯示著癌症病人如欲達到和非癌症病人相同治療效果,應使用較高劑量。

壹、 前言

感染往往是癌症病人產生發病率和死亡率的一個主要原因,而目前革蘭氏陽性菌感染的盛行率又普遍上升,在美國的研究顯示在癌症病人革蘭氏陽性球菌感染的盛行率從62%增加到76%;革蘭氏陰性桿菌從21.5%下降至14%,真菌的感染從15%下降至8%1,通常在治療革蘭氏陽性菌的感染或是癌症病人產生嗜中性白血球低下發燒 (neutropenic fever) 需要使用廣效性抗生素,vancomycin 往往是首選藥品之ㄧ,而 vancomycin 在癌症病人的藥物動力學 (pharmacokinetic) 研究早在1994年西方國家曾有學者發表指出,Le Normand 等人共蒐集10位使用 vancomycin 嗜中性白血球低下癌症病人,發現 vancomycin 分佈體積增加3倍以及較短的排除半衰期,並建議應使用新的治療方案2,而另外 Fernández de Gatta 等人也提出血液惡性腫瘤病人會顯著的影響 vancomycin 清除率,並提出癌症病人適當的建議劑量圖表3,2005年 Buelga 等人也提出在血液惡性腫瘤病人 vancomycin 清除率和分佈體積和非癌症病人相比有稍微偏高4,而2009年東方國家也有類似的研究發現 vancomycin 在癌症病人因分佈體積和清除率的增加,因此應使用較高劑量以達臨床上治療效果1,5,而我國國人是否也有相同情形值得探討,因此本文藉由本院藥物血中濃度監測 (therapeutic drug monitoring,TDM) 結果,回溯性分析癌症和非癌症病人,比較藥物動力學參數差異,以探討我國癌症病人適當的建議劑量。

貳、研究方法

本研究為回溯性之分析,利用某醫學中心之 TDM 資料庫蒐集95年1月至99年3月使用 vancomycin 之癌症和非癌症的成人病人 (年齡≥18歲;並排除加護病房、末期腎衰竭、急性腎衰竭之病人),所有病人接受間歇性 (intermittent) 投藥方式 (QD 或 Q12H、Q8H、Q6H) 給予 vancomycin,且經由靜脈輸注給藥,在第4個劑量以上進行 TDM 監測,並依據監測結果使用一室分佈模式 (one-compartment model) 分別算出分佈體積、清除率和半衰期等藥物動力學參數 (表一),比較兩者之間的差異性,以利癌症病人適當的劑量調整。

參、癌症定義與統計分析

癌症定義:ICD-9 140-239。統計分析:運用電腦 EXCEL 程式使用 student's t-test 做為統計之方法,以 P<0.05 視為有意義之差異。

肆、結果

依據 ICD-9 140-239癌症病人共蒐集50位,癌症分類如表二,非癌症蒐集64位,病人基本資料如表三,所有病人基本特質並無統計上明顯差異 (表三),監測結果顯示癌症病人平均每日 vancomycin 劑量為1785±451.78 mg/day,非癌症病人1789±278.95 mg/day;給藥間隔癌症病人為11.16±4.48 hr,非癌症病人 10.41±2.88 hr,兩者並無統計上差異 (P>0.05),然而癌症病人平均 vancomycin 波峯 (peak) 濃度為29.06±7.39 µg/mL,非癌症病人為34.35±9.95 µg/mL;波谷 (trough) 濃度癌症病人為13.54±6.87 µg/mL,非癌症病人為16.8±8.02 µg/mL,兩者均較非癌症病人為低,並具有統計上差異 (p<0.05) (表四),進一步分析藥物動力學參數,發現癌症病人平均分佈體積為51.22±13.11 L,高於非癌症病人42.66±11.32 L,增加20.06%,並具有統計上差異 (P<0.05);清除率方面同樣癌症病人平均清除率為3.76±1.41 L/h,高於非癌症病人3.08±0.89 L/h,增加22.08%,並具有統計上差異 (P<0.05),然而平均半衰期兩者並無統計上差異。

伍、討論

Vancomycin 在癌症病人的藥物動力學研究目前已有些許的研究報告,而西方國家研究對象大都是針對血液惡性腫瘤病人2-4,7,東方國家研究對象大都泛指所有癌症病人1,5,8,因此本研究經由回溯性蒐集,蒐集癌症對象泛指所有癌症病人,並以 ICD-9 140-239為準則,根據本研究發現癌症病人使用 vanomycin 在每日劑量上和非癌症病人相比均無明顯差異,然而 TDM 監測結果顯示癌症病人 vancomycin 波谷濃度低於非癌症病人 (13.54 ± 6.87 µg/mL vs 16.8 ± 8.02 µg/mL),並具有統計上差異 (表四),這結果也和2010年日本學者 Sadoh S. 等人的研究相符合,Sadoh S. 等人的研究指出共蒐集21位惡性腫瘤病人和28位非惡性腫瘤老年病人,兩者在每日劑量上同樣均無明顯差異,然而監測出波谷濃度分別為10.72 ± 5.37 µg/mL vs 19.39±9.48 µg/mL (P<0.05),也是同樣癌症病人波谷濃度低於非癌症病人5;另外本研究 vancomycin 波峯濃度同樣癌症病人低於非癌症病人 (29.06 ± 7.39 µg/mL vs 34.35 ± 9.95 µg/mL,P<0.05,表四),這也和2009年 Al-Kofide 等人回溯性分析癌症和非癌症病人使用 vancomycin 在每日劑量無明顯差異下,波峯濃度癌症病人明顯低於非癌症病人 (19.1± 6.3 µg/mL vs 27.2 ± 10.9 µg/mL,P<0.05) 相符合1,另外值得一題,根據2009年美國感染相關學會針對 vancomycin 提出建議,建議 vancomycin 波谷濃度應由原來5-15 µg/mL 提升至10-20 µg/mL 以避免產生抗藥性,尤其針對 Staphylococcus aureus 引起的複雜性感染如菌血症、心內膜炎、骨髓炎、腦膜炎及院內感染肺炎等更建議應維持波谷濃度15-20 µg/mL,以達臨床上較佳之治療效果9,因此預估癌症病人欲達波谷濃度和非癌症病人同樣波谷濃度達15 µg/mL 以上,若以癌症病人個別藥物動力學參數公式計算 (表一),適當的建議劑量平均為36.32±13.94 mg/kg/day,約較非癌症病人30.11±6.96 mg/kg/day 增加20.62%,而這或許是因癌症病人分佈體積和清除率增加之故 (表五),而使得劑量也應較非癌症病人增加,至於癌症病人分佈體積和清除率為何會增加,目前詳細機轉仍未明,先前文獻曾提出幾種理論解釋為何 vancomycin 清除率會增加1:(1) vancomycin 主要經由腎絲球過濾排除,然而在一些研究也顯示腎小管分泌也會排除 vancomycin,而癌症病人可能是因此排除途徑增加,而增加清除率,(2) vancomycin 可能某些是經由肝臟代謝 (共軛結合 conjugation) 去活性,而癌症病人可能是因此途徑增加,而導致濃度較低,(3) 癌症病人可能是因給予較多靜脈注射液,而使得尿量增加,降低 vancomycin 再吸收,而增加清除率。上述這些理論目前尚無法證實,然而目前一般文獻建議 vancomycin 在腎功能正常的病人建議劑量為15-20 mg/kg q8h-q12h 仍普遍使用於癌症病人,2009年 Ferna'ndez de Gatta Mdel 等人針對惡性血液疾病病人研究結果顯示,癌症病人需要較高治療劑量以達臨床上治療效果,並建議適當的建議劑量為:vancomycin 1500 mg/day、3000 mg/day、4000 mg/day 相對於 clcr<60、60-120、>120 mL/min7,因此癌症病人使用 vancomycin 應特別注意適當的劑量調整,必要時應以藥物血中濃度監測為依據,以確保病人臨床療效。

陸、結論

此回溯性分析初步結果顯示我國癌症病人使用 vancomycin 同樣會增加分佈體積和清除率,因此癌症病人如欲達到和非癌症病人相同治療效果,應使用較高劑量,然而這仍需要有更多的案例、更進一步的分析,而我們也將繼續朝此方向努力,以提供國人癌症病人最適當的建議劑量。

Pharmacokinetics of Vancomycin in Cancer Patients

Abstract

Several pharmacokinetic studies in cancer patients documented the necessity of dose adjustment for vancomycin user because of the increased clearance and volume distribution. It is valuable to discuss this issue in Taiwanese population. A retrospective study was used from January 2006 to March 2010 in a medical center and data were collected among vancomycin user with or without cancer. Vancomycin serum concentrations were determined after the fourth dose or at steady state. The individual pharmacokinetic parameters (volume of distribution, clearance and half-life) were calculated using a one-compartment model and compared between cancer and non-cancer patients.

A total of 114 patients were recruited in this study and classified into cancer (n=50) and non-cancer (n=64) groups. There were no significant differences between two groups in the interval and daily dosage of vancomycin, but the peak and trough concentration were significantly lower in the cancer group than those in the non-cancer group. Further analysis of pharmacokinetic parameters, the mean volume of distribution (Vd) and clearance (CL) in the cancer group were higher significantly than those in non-cancer group (Means of Vd were respectively 51.22±13.11 L in the cancer group and 42.66±11.32 L in the noncancer group, p<0.05; means of CL were respectively 3.76±1.41 L/h in the cancer group and 3.08±0.89 L/h in the non-cancer group, p<0.05), but the average of half-life was no statistical difference between the two groups. These results implied that higher doses for cancer patients were needed to achieve the same therapeutic effect.
