益生菌改善胃腸道與呼吸道疾病?

摘要

有效的免疫調節功能和抑制病原體菌叢的增殖,使得益生菌成為未來看好的治療方法之一。然而,臨床試驗的結果卻無法在有無使用益生菌的組別觀察到顯著差異。此一現象可由兩方面解讀:一、致病機轉不只一種,益生菌無法藉由影響其中一個機轉,而達到緩解疾病之目的。二、益生菌的種類和使用劑量在不同研究中差異極大,亦會影響實驗結果的判讀。根據目前的研究成果,益生菌的使用的確無法成為主流療法,但可提供臨床照護者多一類藥物選擇。對於較為棘手或病情較為複雜的病人,或許可以考慮給予適量益生菌,有助於改善疾病狀態。

壹、前言

強調具有調節免疫機能的益生菌,隨著最近爆發的「塑化劑」事件,再次成為媒體注意的焦點。由於購買率很高,所以當產品被汙染的消息一曝光,便造成人心惶惶,深怕自己已經成為受害者之一。事實上,益生菌是不是真的具有報章雜誌上所刊載之「神效」?本文透過了解相關藥理作用及機轉輔以臨床試驗的證據,提供益生菌應用在胃腸道疾病及呼吸道疾病的臨床效益。

貳、益生菌簡介

世界衛生組織對於益生菌的定義為「服用足夠分量的狀況下,可以提供宿主健康之活的微生物」。益生菌種類如表一所列,包含 Bifidobacterium、Lactobacillusi…等1。各式菌種與目前已被證實的療效,整理列於表二2。益生菌對於人體的好處在於有效改變腸道菌叢。腸道菌叢是由多種不同菌株組成的生態系統,附著於小腸黏膜 (mucosa)。這些菌叢、黏膜及黏液共同構築黏膜屏障 (mucosal barrier),形成對抗潛在於管腔內免疫性因子之防護系統。其他的正面效果包括:1. 參與小腸壁的形成。2. 抵抗其他菌株的移殖 (colonization)。3. 製造維他命。4. 與黏膜免疫系統 (mucosal immune system) 產生交互作用3。益生菌可有效透過影響小腸上皮細胞 (intestinal epithelial cells, IEC) 和巨噬細胞 (macrophage) 以強化小腸黏膜屏障的功能。此外,益生菌更可有效增進腸胃道系統的功能及調節呼吸道黏膜 (respiratory mucosa) 的免疫反應4

有關於使用益生菌的最適量,仍未有定論。由於益生菌不只一種菌株,不同菌株在不同數量的狀況下才具效果。雖然缺乏劑量相關的研究,但 AFSSA (Agence Francaise de Securite Sanitaire des Aliments) 針對益生菌提出以下幾點說明:1.益生菌的數量是決定體內各腔室是否能獲得高濃度的重要因子。2. 一般認為益生菌濃度在小腸需≥106 cfu (colony-forming unit)/mL、在大腸需≥108 cfu/mL,但立論仍屬薄弱3

參、益生菌與腸胃道疾病

益生菌有益於腸胃道疾病之預後,其對於腸胃道的貢獻包括:1. 促進腸道發展:經實驗證實,Lactobacillus rhamnosus G (LGG) 可以在發育中的老鼠腸道系統內降低細胞凋亡作用和提升具有細胞保護作用之基因表現程度。2. 營養攝取:Lactobacillus acidophilus 藉由分泌可溶性分子可增進小腸吸收電解質的能力。3. 小腸上皮恆定性:益生菌具有促進上皮增生、分泌抗菌物質和與病原體競爭等效果,能有效維持上皮細胞固有功能。4. 小腸免疫反應:益生菌提升先天性免疫 (innate immune) 與適應性免疫 (adaptive immune)5

益生菌臨床應用於許多腸胃道疾病:發炎性腸道疾病、壞死性結腸炎5、抗生素引起的腹瀉、幽門桿菌治療的輔助療法…等等6。接著進一步探討益生菌在 NEC 以及幽門桿菌治療之輔助療法的角色。

一、壞死性結腸炎

NEC 是小腸發炎壞死的病症,通常會造成早產兒極高的死亡率5。已知的風險因子包括:早產、餵養配方奶 (formula feeding)、小腸缺血 (intestinal ischemia) 和細菌增殖 (bacterial colonization)。NEC 可能導致住院時間延長、罹患短腸症 (short bowel syndrome) 和終身神經發展缺陷8。目前疾病的治療以支持性療法或外科手術介入為主7

有鑑於各臨床試驗的差異極大,無法提出一個適合早產兒使用的共識與建議。Deshpande G 等人整合已知的證據,提出早產兒使用益生菌的具體建議8。此研究認為常規給予益生菌對於大部分的早產兒皆是有益的,但不建議給予 ELBW (extremely low birth weight) (出生體重小於1公斤、妊娠週數小於28週) 的早產兒使用。ELBW 的早產兒使用益生菌會增加菌叢轉移 (translocation) 的機會。推薦使用的益生菌株包括 bifidobacteria 和 lactobacilli。合併使用兩種菌株,可增進內生性乳酸菌的增長 (bifidogenic effect)。建議使用劑量則與 AFSSA 的建議相差不遠,劑量大約是106-107 cfu/g。妊娠週數小於28週的早產兒建議使用劑量為3×109 cfu/day;至於 ELBW 的早產兒,建議在一日腸道進食量 (enteral feeding) 達到50-60 mL/kg/day 後,可開始使用1.5×109 cfu/day 的益生菌。適當使用時機則認為越早開始使用越好。可以考慮停用的時間則建議在校正妊娠年齡 (corrected gestational age) 達36至37週8

二、幽門螺旋桿菌治療

幽門螺旋桿菌感染是引起胃潰瘍、十二指腸潰瘍、胃腺癌和胃淋巴癌的主要原因。幽門螺旋桿菌感染之第一線治療方式包括標準三合療法、系列性療法 (sequential therapy)、不含鉍劑之四合療法10。然而,這些療法的失敗率大約10-35%11,但台灣的研究顯示標準三合療法使用7天可達82-94%的除菌率。值得注意的是台灣地區對於 clarithromycin 和 amoxicillin 的抗藥性有逐年增加的趨勢,易造成治療失敗的比率提高10

益生菌可和附著在腸壁的病原菌競爭,也可藉由與上皮細胞的接觸,釋出抑制病原體的物質11。研究指出 Lactobacillus reuteri 和 Weissella confuse 能夠藉由競爭幽門螺旋桿菌之結合位,達到抑制生長的效果12。此外,Wilhelm SM 等人蒐集四篇雙盲隨機試驗的研究,主要結果皆是使用抗生素相關的副作用及其耐受性。結果發現益生菌可以增加耐受性、降低副作用發生率,而除菌率在使用益生菌後未有改變11

肆、益生菌與肺部疾病

越來越多的證據證實口服益生菌可以有效調節腸胃道以外的免疫反應,包括呼吸道黏膜。腸胃道菌叢改變會導致黏膜免疫耐受性 (immune tolerance) 的變化,進而增加過敏性疾病的發生率。根據流病學研究可知腸道中 lactobacilli 和 bifidobacteria 的減少與氣喘發生率的提高有關。隨著抗生素抗藥性的增加,肺部疾病的治療方針開始著眼於調節黏膜免疫反應。呼吸道感染會進一步惡化氣喘及其他疾病的進程,而乳酸菌可以降低在腸胃道及呼吸系統的細菌或病毒感染發生率4。以下分別探討益生菌與各種肺部疾病之關係。

一、 呼吸器相關肺炎 (ventilator-associated pneumonia)

呼吸器相關肺炎的病理機轉包括上呼吸道的病菌增殖、生物膜 (biofilms) 形成導致汙染物進入人體。益生菌的使用可維持上呼吸道的微生物平衡,避免呼吸器相關肺炎的好發。其他益處包括降增強黏膜屏障功能和減少細菌移行13

益生菌應用於人體的臨床研究,可透過呼吸器相關肺炎發生率、死亡率、住院天數、腹瀉與益生菌相關疾病之終點,評估益生菌對於人體的實質好處14。Siempos II 等人分析五份研究結果評估益生菌對人體的實際效益14。結果指出使用益生菌的族群可降低39%的呼吸器相關肺炎發生率。加護病房和住院期間的死亡率亦有降低風險的趨勢。再者,腹瀉的副作用在兩組出現比例皆偏低,無顯著差異。此篇研究的結論證實益生菌的確能有效降低呼吸器相關肺炎的發生率14

二、 院內肺炎 (nosocomial pneumonia)

院內肺炎之風險因子包括:暴露於醫院環境時間及廣效性抗生素之使用。使用鼻胃管等侵入性器械亦會進一步破壞宿主的免疫防禦機制,導致院內感染的出現。理論上,重複使用 lactobacillus 可以抑制口咽部細菌生長15

受限於收案人數少和不同的研究終點,McNabb B 等人蒐集了七篇研究,仍無法提供益生菌使用於院內肺炎足夠的證據力15。結論顯示使用益生菌可以降低病原菌的增殖速率、減少住院天數、減少感染率,但未達統計上的顯著差異。有關於重症病人使用益生菌的安全性考量,必須特別考慮因使用益生菌引起的侵入性感染風險15

伍、結論

透過動物實驗與分子生物學的技術,可以確立益生菌對於調節與改善免疫系統的好處。但是,從臨床試驗的結果卻無法看出顯著的好處。腸胃道疾病和肺部疾病牽涉的病理機轉多且複雜,單純使用益生菌無法有效控制疾病進程。益生菌的使用變異性極大,包括使用量、不同菌株、多重菌株的交互作用都可能會影響其實驗結果。因此目前益生菌依舊停留於「保健」或是「輔助療法」之用,提供病人於正規療法之外更多的藥物治療選擇。唯有透過更完善的研究設計和病人樣本數的提升,才能進一步證實益生菌於疾病治療上的效益。

Probiotics Improving GI and Respiratory Diseases

Abstract

Probiotics, defined as live microorganisms administered in adequate amounts confer a health benefit on the host by FAO/WHO, are promising agents on both gastroenteral (GI) and respiratory diseases. They can effectively turn the originated and comprimised intestinal microbs into normal and healthy ones. Intact microbiota can further strengthen the function of mucosal barrier, manifesting as intensifying the immune response to xenobiotics or eliminating the toxic pathogen. Its benefits come from effectively altering the mucosal immunity and preventing pathogen from colonizing. Many diseases are related to devastating immune function, including necrotizing enterocolitis (NEC), H. pylori infection, and pneumonia.

Theoretically, we can expect the recovery or delayed progression on diseases after administration of probiotics. But, in fact, according to the end points of clinical trials, it is hardly delineated that the probiotics are significantly beneficial on diseases. Complicated pathological mechanisms of various diseases and imperfect study design are suspected the possible causes.

In conclusion, the roles of probiotics remain as adjunctive therapy on therapeutic consideration. It is necessary to conduct further studies and well-designed trials to support rational and regular use of different probiotics. In particular, based on the results available so far, pharmacists play an important role on providing correct concepts and intact information to the general population.
