Patisiran 治療遺傳性澱粉樣蛋白疾病

摘要

遺傳性澱粉樣蛋白疾病 (hereditary transthyretin amyloidosis, hATTR) 是罕見的由於血清前白蛋白 (prealbumin 又稱轉甲狀腺素蛋白 transthyretin, TTR) 基因突變所造成的體染色體顯性遺傳疾病。在正常情況下 transthyretin 是由四個單元體 (monomer) 結合而成的四元體 (tetramer) 結構1,由於基因突變,導致四元體結構不穩定容易分解成單元體,此單元體經過錯誤折疊後聚集堆疊形成澱粉樣蛋白纖維 (amyloid fibrils),沉積在身體眾多的組織、器官,包括周邊神經系統、心臟、腎臟與腸胃道,最終逐漸導致病人失能與死亡。

Patisiran 是一種雙股小干擾核醣核酸 (small interfering ribonucleic acid, siRNA),作用在調控 transthyretin 的訊息 RNA (mRNA),降低肝臟生合成的野生型 (wild-type) 或突變型 transthyretin 的產生。美國食品藥物管理局於2018年8月10日核准 patisiran 用於成人因遺傳性澱粉樣蛋白疾病所導致的多發神經病變,此藥亦是第一個美國 FDA 核准的小干擾核醣核酸藥品。

壹、前言

遺傳性澱粉樣蛋白疾病是一種致命性的體染色體顯性遺傳疾病,一旦確診罹病,其後代無論男女每一位都有百分之五十的遺傳機率,以往的治療方式只有肝臟移植以及使用小分子穩定劑來穩定 transthyretin 分子結構,但都無法有效的停止疾病的進展2,一旦病徵開始,只能存活2到15年。近年來,隨著醫藥科學之進步,遺傳性澱粉樣蛋白疾病之治療出現了一線曙光,一個第三期臨床試驗使用小干擾核糖核酸藥品 patisiran 造成核糖核酸干擾 (RNA interfering)3,4,形成所謂基因沉默 (gene silencing) 現象,讓負責生合成 transthyretin 的異常基因無法表達,可降低病人血中異常的 transthyretin 的產生,及其後續形成的澱粉樣蛋白纖維於組織器官的沉積,有效控制並延緩病情,改善了病人的生活品質。

貳、Transthyretin 與遺傳性澱粉樣蛋白疾病

Transthyretin 是一種在體內主要負責運送視網醇結合蛋白 (retinol binding protein) 與thyroxine (T4) 的蛋白質,主要在肝臟生合成,並分泌到血漿中;少部分生合成於腦部的脈絡叢 (choroid plexus) 並分泌到腦脊髓液內5。Transthyretin 是由四個單元體結合而成的四元體結構,每個單元體是由127個胺基酸所形成的胜肽,一旦調控 transthyretin 基因產生突變,會使得單元體的其中的一個胺基酸被置換。一個突變的 transthyretin 可能由1-4個異常單元體所組成2

調控 transthyretin 生合成的基因位於第18對染色體18q11.2-q12.1位置,目前已在 TTR 基因上找到將近120個相關的突變點位置,其中以 Val30Met (第30個氨基酸由原本的 valine 突變為 methionine) 為最常見的突變,產生這類突變的人口主要分布於葡萄牙、瑞典與日本。而非洲裔美國人則較多產生 Val122Ile 突變,有幾篇台灣的研究則指出,台灣有獨特的 Ala97Ser 突變6,7

產生遺傳性澱粉樣蛋白疾病的主因就是調控 transthyretin 基因突變後,所產生的 transthyretin 因結構不穩定,容易分解成單元體,單元體經過錯誤折疊後,聚集堆疊形成澱粉樣蛋白纖維,沉積在體內眾多的組織器官,如周邊神經系統、心臟、腎臟、眼球玻璃體與腸胃道,最終逐漸導致病人失能與死亡的疾病。若疾病主要造成週邊及自主神經病變,則也稱為家族性澱粉樣多發性神經病變 (familial amyloid polyneuropathy, FAP)。

參、Patisiran 的作用與臨床試驗

Patisiran 是一種由脂質奈米微粒 (lipid nanoparticles, LNPs) 所包覆的雙股小干擾核醣核酸,每股各有21個鹼基,其中3'端各有兩個未配對鹼基。Patisiran 經由靜脈輸注通過血液循環進入肝細胞後,鍵結於細胞質內的 RISC (RNA-induced silencing complex),經過 RISC 的剪輯作用保留了 antisense strand (反義股),此反義股鍵結於突變型或野生型 transthyretin 訊息 RNA 的3'UTR (3' untranslated region) 的配對位置,再經由 RISC 內的 AGO2 (argonaute 2) 單元的 endonuclease 將訊息 RNA 裂解,使之無法繼續轉譯生合成突變型或野生型 transthyretin (圖一)1,3,達到治療遺傳性澱粉樣蛋白疾病的目的。

有關 patisiran 最重要的試驗是名為 APOLLO 臨床試驗8,9,它是一個隨機、雙盲、具有安慰劑對照組的跨國性第三期臨床試驗,共收入225個罹患 hATTR 成年病人,全體病人以2:1分組,實驗組 (N = 148) 每三週一次以注射方式依照體重投與 0.3 mg/kg patisiran,對照組 (N = 77),歷時18個月。試驗的主要終點為改良神經病變損害評分 (modified Neuropathy Impairment Score+7,mNIS+7),分數越高代表神經損傷越嚴重。次要終點有數個指標,如運動神經強度 (motor strength, NIS-weakness)、失能評分 (disability, rasch-built overall disability scale)、營養狀態 (nutritional status, modified body-mass index,mBMI)、步行速度 (gait speed, 10 m walk test),其中最重要為Norfolk糖尿病神經病變生活品質評分 (Norfolk Quality of Life, Norfolk QoL-DN),分數越高代表生活品質越差。

試驗結果的有效性部分,實驗組比起對照組的 mNIS+7評分差值達-34.0分 (P < 0.001),Norfolk QoL-DN 差值為-21.1分 (P < 0.001),10公尺步行差值為0.31公尺/秒 (P < 0.001),mBMI 差值為115.7 (P < 0.001)。從上述的結果看來,實驗組的主要和次要終點均得到顯著改善。

試驗的安全性部分,兩組發生率較高的不良反應為腹瀉、末梢水腫、跌倒、噁心、輸注相關的反應及便秘,其中實驗組發生率高於對照組的不良反應為末梢水腫 (30.8% vs. 22.1%) 以及輸注相關的反應 (18.9% vs. 9.1%)。實驗組和對照組整體不良事件發生率相似 (AEs:96.6% vs. 97.4%),嚴重不良事件發生率也相似 (SAEs:36.5% vs. 40.3%),死亡率 (4.7% vs. 14.3%),因不良反應導致退出試驗的比例為 (4.7% vs. 14.3%)。

肆、未來展望與結論

隨著醫藥科技的快速發展,基因缺失或突變造成的疾病不再是不治之症,目前 siRNA 與A.S.O (antisense oligonucleotide) 兩類藥品可專一性的造成特定基因沉默,讓細胞質內特定 mRNA 裂解,無法轉譯而製造出異常的蛋白質。siRNA 的運用廣泛,已有有關於黃斑部病變、實體腫瘤的治療、抗病毒、降低低密度脂蛋白等數個臨床試驗正在進行,期待臨床試驗具有良好效果與安全性,造福更多病人。

 

Patisiran for the Treatment of Hereditary Transthyretin Amyloidosis

Abstract

Hereditary transthyretin amyloidosis (hATTR) is a rare, autosomal dominant disorder caused by mutations in the transthyretin (TTR) gene. Normally, TTR is a tetramer made up of 4 single-chain monomers. TTR gene mutations are thought to destabilize the protein and cause tetramer dissociation into monomers, which aggregate into amyloid fibrils. These amyloid fibrils then accumulate in multiple organs, including the peripheral nervous system, heart, kidney, and gastrointestinal tract. Finally, it’s gradually leading to significant disability and death.

Patisiran is a double-stranded small interfering ribonucleic acid (siRNA) that targets a sequence within the transthyretin messenger RNA that is conserved across wild-type and all TTR variants to decrease hepatic production of mutant and wild-type TTR. In August 10th 2018, the U.S. Food and Drug Administration approved patisiran infusion for the treatment of polyneuropathy caused by hereditary transthyretin-mediated amyloidosis in adult patients. It is also the first FDA approval of a new class of drugs called small interfering ribonucleic acid (siRNA) treatment.
