婦女產後及哺乳期間避孕之觀念

壹、前言

全世界平均每年超過十億的婦女生產,這些婦女產後立即面對如何哺餵嬰兒及避免再次的懷孕1。哺乳在開發中國家是非常重要的事情,除了提供嬰兒營養,促進母子雙方的健康,同時是最好的避孕方式。研究證明母乳提供嬰兒優異的營養並且可對抗多種的感染疾病,例如下呼吸道感染、中耳炎,降低母親產後出血,促進子宮快速復原及排卵延後。此外,哺乳避孕造成生產間隔的延長亦有益於母親及子女的健康2

世界衛生組織(WHO)依據最新的實證回顧公佈之”Global Strategy for Infant and Young Child Feeding”,建議六個月內的嬰兒應完全哺餵母乳,之後添加副食品並持續哺餵至少至兩歲3。完全的哺餵母乳在產後6個月內避孕效果可高達98%,1年內亦可達94%4。國外研究報告發現生產後六週回診的婦女,其中50%沒有完全的哺乳5。目前國內極力推廣哺餵母乳並期望達到最好的效應,但由衛生署委託各學界調查資料中發現6,民國74年產後一個月混合哺餵率佔32.5%,民國78年產後一個月混合哺餵率佔21.2%,純母乳哺餵率僅佔5.4%。有鑑於國內哺乳率過低,民國80年後衛生署保健處推廣母乳哺育計劃。民國93年調查發現嬰兒滿月時純母乳哺育率為33.21%,至六個月時則降為13.13%。哺乳期是否能有效避孕,有許多必要的條件,除哺乳是否完全,哺乳期長短,另包括無月經、正確哺乳等因素配合。因此本文主要針對哺乳生理、哺乳無月經避孕法(LAM)的觀念、及目前產後及哺乳期如何早期避孕加以討論。

貳、哺乳生理

產後授乳主要受到泌乳激素及子宮收縮素的影響7。在懷孕的過程中,泌乳激素可刺激乳房alveolar cell產生乳汁及乳房的成長,而胎盤所產生黃體素則可干擾泌乳激素作用在乳房上alveolar cell的接受器,直接抑制乳汁的產生,同時雌激素的濃度也可阻斷泌乳素的活性。但產後,由胎盤產生的黃體素快速的降低因此無法抑制泌乳激素。泌乳激素持續刺激乳汁的產生加上子宮收縮素使乳汁射入導管,因此引發最初的泌乳,3-4天後母體內的黃體素、雌激素已被清除,乳汁開始分泌。之後,嬰兒的吸吮刺激泌乳素的增加,這對於維持乳汁量是很重要的因素。斷奶後泌乳素濃度降低,約在14-30天內重新恢復排卵功能。

參、 哺乳無月經方法(LAM)之觀念及有效性

哺乳避孕的觀念就是哺乳無月經方法(lactational amenorrhea method;LAM),LAM是產後最常使用的過渡期避孕方法。機轉是靠哺餵母乳時造成泌乳素濃度增加,抑制下視丘脈衝式的分泌GnRH(gonadotropin-releasing hormone),干擾下視丘-腦下垂體-卵巢系統(hypothalamic-pituitary-ovarian axis),間接的抑制濾泡刺激素(FSH)與黃體素(LH)進而抑制排卵,但是仍有許多的因素會影響其避孕的有效性。

LAM在1988年於Rockefeller Bellagio Conference Center中提到,適用於計畫哺乳六個月以上的婦女,不論產前及產後醫療人員都需要與她充分的討論、周全的計畫。為確定LAM的有效性,產後需隨時評估三項標準以瞭解目前可能懷孕的機會,是否仍可單獨使用LAM或必須配合使用其他避孕方式(圖一)8

第一項:是否恢復經期?此項最重要,經期恢復則表示排卵可能性增加。

第二項:是否規律的提供副食品、配方奶,或是否長時間(白天或晚上)未能哺餵母乳?嬰兒使用配方奶及副食品,即使尚未有月經的婦女,都可能增加排卵及懷孕的機會。為達有效避孕,哺餵母乳時須注意幾點:1.規律,白天餵食間隔不超過4小時,晚上不超過6小時。2.配方奶或副食品不超過進食總量的5-10%。另外,使用手或幫浦擠奶對母親內分泌的反應較吸吮為弱,亦可能增加排卵及懷孕的機率。

第三項:嬰兒是否超過六個月大?2005年National Immunization Survey, CDC and Prevention報告指出,產後7天完全哺乳的比率為59.4%,產後42天為46.4%9;由此可知,雖然產後6個月內完全哺乳避孕效果可達98%,如符合前兩項則1年內亦可達94%4,但實際上完全哺乳的比率低並隨時間遞減,且哺乳期間多數婦女並無法符合LAM的注意事項,如:產後6個月內經期恢復、臨時停餵母奶,所以目前哺乳婦女,並無法發揮如預期的避孕效果。未哺乳的婦女更無法預期何時經期及排卵回復。因此,產後不論是否哺乳,早期避孕在目前都是非常的重要。

肆、避孕方法的選擇

避孕方法可分為非荷爾蒙療法及荷爾蒙療法。在哺乳期使用非荷爾蒙是最合適的,如:哺乳無月經方法(Lactational amenorrhea method;LAM)、可逆性避孕法(保險套、子宮內含銅避孕器)、永久性避孕法(輸精管或輸卵管結紮),因為荷爾蒙可能會干擾泌乳並且經由乳汁進入嬰兒體內,可能增加嬰兒的風險,但目前有許多文獻證明單獨使用黃體素避孕藥對母親及胎兒皆無影響,因此,如何選擇合適的方式極其重要。

伍、非荷爾蒙療法

哺乳期除哺乳無月經方法(LAM)及男性保險套外,最廣泛使用為子宮內含銅避孕器。它具有良好的避孕效果,並已證明哺乳時子宮收縮並不會增加排出機率、銅離子濃度亦不會影響乳汁。其他如採取輸卵管結紮需全身麻醉,必須通知麻醉科醫師選擇最合適的麻醉劑。手術後母親必須完全清醒才可哺乳,使用短效的Benzodiazepine(如Lorazepam)、肌肉鬆弛劑、吸入性藥物、及局部麻醉劑可能是安全的,但避免使用Meperidine及長效型Benzodiazepine(如Diazepam)。哺乳的婦女可能喜歡自然避孕方式,觀察授乳期子宮頸黏液的改變可做為是否具生殖力的指標1。這些非荷爾蒙的方式不影響授乳,但仍應優先考量其合適性。

陸、荷爾蒙療法

授乳婦女一般皆以非荷爾蒙避孕方式為主,但近年來隨者荷爾蒙避孕療法的發展,有成份上的改變(以黃體素為主),劑型上的改變(如口服、長效針劑、植入型、子宮內治療),到避孕時間的改變(24小時到5年不等),使哺乳婦女開始有了更多的選擇。

授乳婦女使用荷爾蒙療法避孕目前仍有爭議,主要原因有二。第一、擔心嬰兒經由乳汁攝入荷爾蒙,加上嬰兒肝臟的不成熟使藥物無法完全代謝,腎臟的不成熟可能造成排泄不全,導致血液循環中出現高濃度具活性的荷爾蒙進而傷害嬰兒。但研究中發現新生兒在早期並無法吸收來自乳汁的荷爾蒙,隨著新生兒週數增加吸收可能增加,但排泄能力也增加10。WHO在多國前瞻性研究中追蹤2466位哺乳母親於產後6周開始使用單獨含黃體素避孕藥的嬰兒,研究發現他們在發育與成長上並無不良影響11。第二、荷爾蒙避孕藥含女性素及黃體素,可能同時影響泌乳素在乳房的作用,而減弱或干擾泌乳。此外,生產後早期血液處於高凝血狀態,不應使用含有雌激素的製劑。

許多研究證明單獨使用黃體素的避孕製劑,包括口服藥Mini Pills®(norethindrone)、避孕針Depo-Provera®(DMPA;Depot medroxyprogesterone acetate)、植入緩釋Implanon®(etonogestrel)及子宮內黃體素緩釋劑Mirena®蜜蕊娜(levonorgestrel)等,雖然使用在哺乳早期仍有爭議,但對於泌乳及胎兒目前並沒有發現不良反應12。WHO研究報告產後六週單獨使用口服黃體素避孕藥與使用子宮內避孕器,6週後兩組之間並無降低乳汁分泌量,但18週後使用黃體素者降低乳汁分泌量12%,而使用子宮內避孕器者只降低6.1%13。另有研究證明含黃體素的避孕方式不論劑型是子宮環、皮下植入或口服都可延長哺乳無月經期,達到有效的避孕,但也可能發生不規律的出血,導致少部分的婦女因為出血問題而停止使用此避孕方式14

近年來各國推廣哺餵母乳,開始有文章評估授乳期合併使用或單獨使用黃體素避孕藥之間的差異,目前仍無強而有力的資料可以評估對母親乳汁量及品質的影響,現有的資料也無法建議哺乳期是否可以使用合併型荷爾蒙的避孕方式。然而,許多綜合回顧性文章發現使用單獨黃體素避孕方式並沒有証據顯示對嬰兒的成長有不良的反應12。表一為美國婦產科醫師協會(ACOG;American College of Obstetricians and Gynecologists)及WHO建議婦女在各種身體狀況下優先選用的避孕方式,其中包含哺乳婦女避孕方式的選擇。

產後婦女何時開始使用荷爾蒙避孕目前仍有爭議,多數專家認為哺乳婦女產後6週再開始使用單獨含黃體素者,不建議使用含有雌激素。如需使用合併荷爾蒙避孕,因考慮到產後高凝血狀態,WHO建議即使未哺乳婦女也應於產後3週,哺乳婦女則至少產後6個月才可使用,並須評估哺乳的技巧及方式是否正確15。臺灣目前單獨含黃體素製劑者上市有口服藥、針劑及子宮內避孕器,每種劑型各有特色,使用方式及時機也有各別差異,除醫師建議外,仍以藥廠提供的仿單資訊(prescribing information)為主。

柒、結論

有鑑於婦女就業率增加,職業場所的不便,乳房脹痛等諸多因素,都可能導致哺乳方式改變、提早停止哺乳。另外,使用配方奶及副食品比例的增加都會使哺乳婦女提早恢復懷孕能力。早期避孕除選擇非荷爾蒙避孕方式,黃體素避孕方式並不會影響嬰兒的成長與發育,目前也是非常合適的選擇。所有醫療人員必須瞭解哺乳方式不同於早期,避孕方式也有更多選擇,為避免產後婦女不預期的再次懷孕,需在產前照護的最後階段評估準媽媽們產後避孕的需求,鼓勵哺乳的同時必須告知正確的哺乳觀念及搭配其他避孕方式,更重要的是依據準媽媽個人喜好及生活習慣協助擬定周全的避孕計畫。
